2025 Skilled Nursing Facility Enrichment Day

Este evento presencial GRATUITO presentará temas que incluyen:

• Goals of care
• Insuficiencia cardíaca
• Enfermedad pulmonar obstructiva crónica
• Neumonía
• Diabetes
• Transiciones al hospital/SNF
• Intervenciones de salud conductual

Attendees will be able to demonstrate knowledge of pathophysiology of disease, interventions, POLST forms and goals of care, safe transitions between hospitals and post-acute facilities, and use of respiratory equipment, all aimed at improving health of individuals in long-term care as demonstrated by nursing participants obtaining a post-test score of >=75%.

Target Audience: Nursing home staff and clinicians including RNs, LPS, social workers, administrators, and ancillary staff interestd and involved in process and quality improvement.
